A submalar implant is a solid prosthetic inserted beneath the cheekbone to restore volume and definition to a sunken or flat midface. The implant is typically made of silicone or other biocompatible material and is placed through an incision inside the mouth or lower eyelid. It provides permanent structural augmentation to the cheek and lower facial region.

Candidates include people with naturally low cheekbones, age-related volume loss in the cheeks or under-eye area, or facial asymmetry they wish to correct. A qualified surgeon will assess bone structure, skin quality, and aesthetic goals to determine if an implant is appropriate or if alternatives like fillers might suffice.

The surgery is performed under local anesthesia with sedation or general anesthesia. The surgeon creates an incision, usually inside the mouth, to form a pocket beneath the cheekbone where the implant is positioned and secured. The procedure typically takes 30 to 60 minutes.

Initial swelling and bruising peak within the first few days and gradually subside over two to three weeks. Patients are usually advised to avoid strenuous activity, heavy lifting, and contact sports for several weeks. Pain is typically mild to moderate and managed with prescribed or over-the-counter medication.

Results are visible immediately but continue to improve as swelling resolves over weeks. The implant provides permanent augmentation, though natural aging and changes in surrounding tissues continue over time. Some people choose removal or revision years later if aesthetic preferences shift.
